What services do we provide?
The Foundation’s dementia programs are developed as a comprehensive, community-based service with a special emphasis on vulnerable, older individuals with the greatest economic or social need, and particularly for low-income minorities. With a stronghold in the Hispanic and urban communities all programs are offered in English and Spanish. Our core programs include Memory Screening/Evaluations Testing, Support, Crisis Management, Education, Long Term Care Planning, Advocacy and Outreach.
What does building a Culture of Health mean to Act Now Foundation Dementia Resource Center as an organization?
