Our mission is to meet the healthcare needs of women, infants and children in NJ while coordinating education, outreach and advocacy through regional planning and collaboration. The consortium conducts extensive community outreach and educational programs for the consumers and health care providers in the state of New Jersey. These include the Community Health Worker Program, Home Visitation Programs, Central Intake, along with many other MCH programs.
PMCH is overseeing the Healthy Women Healthy Family Initiative CHW program in 5 counties including Hudson County. HWHF also incorporates a Black Infant Mortality (BIM) component. The HWHF program and BIM activities focus on reducing health disparities in target regions and cities in NJ (including Jersey City). We look forward to connecting with maternal/child health and community organizations to facility the HWHF and BIM Initiatives.
