What services do we provide?

We work with youth ages 5-21 and their families to coordinate services and supports for youth with mental health, substance abuse and developmental challenges. We are a private, not-for-profit, and our mission is to help youth stay at home, in school and in their community. We also operate a community outreach center (The C.O.V.E. at 312 Pacific Avenue in Jersey City) that hosts events, workshops and meetings that benefit the community.

What does building a Culture of Health mean to Hudson Partnership Care Management Organization as an organization?

We look at supporting the community from a public health perspective, which involves awareness raising, prevention and intervention. We believe the youth that receive services and support from the CMO will be more successful if we help to create a community environment that promotes health and safety from a trauma-informed approach and provides safe spaces for youth to thrive.
